Our doctors and nurse practitioners visit residents where they live, helping to improve access to timely, reliable, and personalised healthcare. We work closely with residents, families, nursing staff, and aged care teams to support ongoing health needs, manage chronic conditions, review medications, and provide responsive medical care.

Dr Aida Vizcaya Guarin is an experienced medical practitioner with more than 45 years of clinical experience across Australia and Colombia.
With a strong focus on aged care medicine, she provides comprehensive medical care for older adults living in residential aged care facilities and the community. Her clinical interests include healthy ageing, frailty, dementia care, chronic disease management, medication optimisation, falls prevention, advance care planning, and palliative care. Dr Vizcaya Guarin is passionate about helping older Australians maintain their health, dignity, independence, and quality of life.

Meet Our Nurse Practitioner
Sandun Hathiringe is an experienced Nurse Practitioner with advanced qualifications from Queensland University of Technology and a strong background in aged care. He has extensive clinical experience supporting older adults and a special interest in gerontology, healthy ageing, chronic disease management, dementia care, falls prevention, medication optimisation, frailty, mental health, and palliative care.
Sandun provides compassionate, evidence-based, and person-centred care tailored to the individual needs of each resident. He works closely with residents, families, nursing staff, and the broader healthcare team to promote comfort, dignity, wellbeing, and the best possible quality of life.

Billing
Eligible consultations provided within Residential Aged Care Facilities are bulk billed through Medicare.
Any services that may attract an out-of-pocket fee will be discussed prior to the consultation.
Communication
Communication regarding residents is generally coordinated through the nursing team at the residential aged care facility to ensure concerns are appropriately triaged and managed. This approach supports timely communication and allows urgent matters to be escalated when required.
For urgent clinical concerns, families and representatives should contact the nursing team at the residential aged care facility directly.
Test Results and Correspondence
All incoming pathology results, imaging reports, specialist correspondence, and hospital discharge information are reviewed as part of the resident's ongoing care.
Where clinically appropriate, results and correspondence will be discussed at the resident's next scheduled review. If urgent follow-up is required, Dr Guarin will arrange review through a facility visit, telehealth consultation, or other appropriate clinical pathway depending on the circumstances.
After-Hours Care
After-hours medical care is available through the facility's established after-hours arrangements and medical support services.
Residents and families requiring urgent assistance outside of normal operating hours should contact the residential aged care facility, which will coordinate the most appropriate after-hours medical service based on the resident's needs.

Telehealth appointments are available where clinically appropriate, providing convenient access to medical care and follow-up support.

This includes working alongside services such as RADAR, OPEN, and SPACE to support comprehensive assessment, coordinated care planning, and timely access to specialist advice when required.
